The aim of treatments such as psychotherapy, physical therapy and hypnotherapy is to prevent involuntary muscle contractions that occur during intercourse. In other words, it aims to relax and control the pelvic floor muscles of the woman. Dr. Ünal provides relaxation of the muscles at the vaginal entrance with the neuromodulator application.

Involuntary contractions of the muscles around the vagina is the cause of vaginismus. The aim of the treatment is to relax the muscles there. We apply neuromodulator drugs to the muscles that cause vaginismus to relax. This treatment is applied in many centers around the world.

What is vaginismus? Who experience it?

Vaginismus is the inability to have intercourse due to excessive and involuntary contraction of the pelvic floor muscle, the muscles around the vagina. The most common symptom is the contraction of the muscles around the vagina and the obstruction of the penile entry into the vagina. As a result of the contraction of the muscles at the vaginal entrance, the woman feels severe pain. The muscle contraction can be so severe that when the woman and her partner try to have intercourse, there is a wall-like structure at the entrance of the vagina and the entrance is interpreted as closed. In fact, many couples think that the structure that closes the vaginal entrance like a wall is the hymen, so they have surgically removed the hymen. However, since such procedures do not eliminate muscle contraction, there is no improvement in complaints.

You may have heard on many platforms that vaginismus is more common in women with low socioeconomic status, but all women can develop vaginismus. This has nothing to do with socioeconomic level.

 

What is the mechanism of action of the neuromodulator used?

The drug used in this treatment is a neuromodulator, that is, it acts on nerve cells. The stimulus that comes out of the woman's brain and gives the order for involuntary contraction comes to the nerve, but since the transmission in the nerve is stopped with medication, it is not transmitted to the muscles around the vagina and there is no contraction in the muscle. In other words, a woman cannot contract these muscles even if she wants to. This removes the "wall" causing the problem. After the neuromodulator application, the woman will be able to have intercourse painlessly and easily.

How is vaginismus treated with a neuromodulator?

 

We do vaginismus treatment under light sedation. During the application, the woman does not feel any pain as she is sleeping. We apply neuromodulators to many points in the muscles around the vagina. The dose of the drug used is very important for the success of the treatment. Adequate dose must therefore be administered in full.

 

After the neuromodulator injection, we administer local anesthesia to the same muscles. Thus, the muscles here become numb for 2-3 hours. Finally, we insert a dilator into the vagina before the woman wakes up. When the patient wakes up from anesthesia, she starts exercising with this dilator, a plastic prosthesis. Since this area is anesthetized, she does not feel pain during the exercise with the dilator. The dilator exercise is done for about 30 minutes. Thus, the woman can perceive that there is no wall or membrane covering the vagina. She realizes that the reason for this “closed” feeling is her own muscles. With this exercise, women get rid of her fears and anxieties.

 

When should the first intercourse after the procedure be?

About 10 days after the vaginismus treatment, the woman feels that she can no longer contract her pelvic floor muscles. After this period, she can try to have intercourse. It is quite normal for the woman to be anxious and excited in the first relationships due to the painful intercourse experience she has experienced before.

After 15 days, the woman must try to have intercourse every day. Our advice to the spouses during this period is to calm down the partner and continue to the intercourse. We suggest not to stop the intercourse. Anxiety may develop in some men due to previous experiences with their partner, which causes the loss of erection. In this cases, we recommend to use of drugs that increase or prolong the erection.

After vaginal relaxation, the woman should try have intercourse everyday and work with a dilator for at least 1 hour everyday. 90% of treated couples are able to have intercourse at the end of the first month. Other couples usually manage to have intercourse up to 3 months.

 

Vaginismus Treatment

 

Vaginismus is one of the first diagnoses that comes to mind when it comes to gynecological diseases because it is seen in most women. Vaginismus is the physical condition of the body, it prevents the penis from entering the vagina even though women want to have sexual intercourse. This situation not only causes major problems in the relationship, but also causes women to develop major psychological problems. However, diagnosing and treating vaginismus is no longer as difficult as it is used to be.

 

How long does the solution last?

A permanent solution to vaginismus is provided with a neuromodulator. Who can be treated for vaginismus?

It can be applied to all women who have never had intercourse before, that is, who have prime vaginismus or who have pain at the beginning of intercourse and therefore avoid intercourse. Women who have received psychotherapy or other treatments before, who have had an anesthetized waist but have not healed, who got no result from hymen removal, all benefit from our vaginismus treatment. Removal of the hymen does not cure vaginismus. Because the problem here is not caused by the hymen. Vaginismus is related to the muscle tissue around the vagina. When the woman cannot have intercourse after the hymen is removed, the woman falls into despair and this situation makes her relationship with her husband worse.

We do not recommend having intercourse with the application of anesthesia or having intercourse by anesthetizing the lower part of the waist. In the meantime, even if the woman has intercourse, when the numbing effect wears off, the pelvic floor muscles contract again and the problem continues. So this treatment has no permanent effect.

The neuromodulator is only effecting the injected muscles. It will not effect the uterus, ovary, bladder or anal sphincter. It will not cause any side effects like urinary incontinence or anal incontinence. The injector’s knowledge and experience has paramount importance for the success of the procedure.

Neuromodulator effect will settle in 14 days. After 14 days you will start to have sex. They stay at the muscles for 6 months. After the effect resolves you may start to contract the perineal and deep pelvic muscles again. Since your brain will forget to over-contract the pelvic muscles, you will never need another treatment. The effect will be permanent. 90% of our patients achieve sexual intercourse within one month. %97 of our patients achieve pain free sex within 3 months.

For your partner, it may be difficult to have erection or continue erection at first weeks. (S)he also needs adaptation to your new situation. Dr. Ünal may prescribe some medication to your partner to help him/her continue the erection and prevent premature ejaculation.

During the operation

Vaginismus treatment with neuromodulator injection will last in 30 minutes. After the procedure you may continue your daily life. If you are coming to Istanbul from abroad, you can travel back home at the same day after the procedure.

Dr. Ünal is performing neuromodulator injection to the pelvic floor muscles under sedation to treat the vaginismus. Woman would not tolerate the injection without sedation. After you will be sedated, you will be placed in the lithotomy position, and Dr. Ünal will inject the neuromodulator in the right muscles around the vagina. Then, she will place a vaginal dilator in the vagina while you are sleeping. When you wake up from anesthesia, you will do exercise with dilator for 30 minutes. The whole procedure will take 1 hour. After the procedure you may continue your daily life.

Only one session of neuromodulator treatment will help you. You do not need to repeat the procedure. You may come to our clinic alone, we do not need your partner.
